I imagine the people interested in a virtual world would mainly join in order to communicate with (and I use the term "communicate" loosely here

) other people.
Think of it not so much as a traditional passive porn experience, but more like a mix of advanced chat room, online dating service, 3D "live cams" and ample oppurtunity for cybersex.
I don't really think it's something I would be particularly interested in either - I rather like my my good old passive porn watching routine

(though I have been messing around with a few single player 3D "sex games/sims", like 3D Sex Villa) - but I imagine many would be.
Otherwise there would probably be substantially fewer strip and sex clubs, virtual prostitutes and sexy outfits, animations and items in Second Life

.
And of course Red Light Center - which was mentioned in this thread - does much the same, just entirely focused on sex rather than also (and probably originally intended) all the regular non-adult stuff in Second Life.
If you want to know what goes on in the minds of people interested in virtual worlds like those, their respective forums can probably help clue you in.
Looks like the Red Light Center forum is actually fairly active with well over 50,000 posts (and obviously Second Life a whole lot more).
